Text Message Consent Form

This form is optional. Crew members opt in by replying YES to the confirmation text Crewzi sends them — that texted confirmation is the consent of record (see how opt-in works). Some employers also like to keep a signed paper record; this is the standard form for that. It is not required to add someone to the crew.

Consent to receive work-scheduling text messages

I am providing my mobile phone number to my employer  (the “Company”) and I agree to receive work-related text messages from the Company through the Crewzi platform, including job assignments (job name, date, time, and job-site information), schedule reminders, and messages from the office about my work.

  • Message and data rates may apply.
  • I can opt out at any time by replying STOP to any message, and get help by replying HELP.
